After clicking one of the patient portal options in the blue box you can create an account or login if you have already created an account.
Once you have created an account you will be able to request an appointment, new patients will be able to fill out all four health forms we need and current patients can update any changed information since last appointment. You will also be able to pay your bill with a credit card (you will need your account number off of the statement mailed to you).
There is also an option to ask our staff non-emergency questions and have them email you back. When you have a message from our staff you will get an email notification to login to your account and check your inbox on the patient portal.
